Your DetailsName* First Last Email* Select your course Confined Space Training EquipmentModel* Serial number* BA set number* ChecksVisually and physically inspect the face mask and demand valve:* • Check the orinasal cup and inhalation valves are seated correctly. • Check the mask visor is free from distortion, cracks and is seated correctly in the frame of the mask. • Check the inside of mask is clean and free of debris and foreign matter. • Check the 5-point rubber head harness straps are undamaged and fully extended. • Check the demand valve is parked. • Check the demand valve O-ring is in place and correctly seated. • Check the demand valve release button. • Check and confirm the demand valve and face mask connection by reconnecting the demand valve to the face mask and gently tugging on the connection. ✓ Pass ✘ Fail Visually and physically inspect the Back Plate:* • Check for cracks in the plastic construction. • Check for deformities indicating heat distortion. • Check for discolouration indicating heat distortion and weakness. • Check the lumbar padding to ensure it is locked in place. ✓ Pass ✘ Fail Visually and physically inspect the sets buckles, cylinder, shoulder and waste straps:* • Check for cuts and tears. • Check for excessive stretching. • Check for deterioration from rot, mildew or UV exposure. • Check the stitching for cut or worn threads. • Check the buckles and adjusters for distortion and cracking. • Check that the straps are extend so that the set can be easily donned. ✓ Pass ✘ Fail Visually and physically inspect the sets airlines:* • Check the recessed airlines in the back plates to ensure they are secure. • Check the high-pressure airline to pressure gauge for cuts and damage. • Check the medium pressure airline to the demand valve for cuts and damage. • Check the cylinder regulator and ensure the black O-ring is in place. ✓ Pass ✘ Fail Visually and physically inspect cylinders:* • Check for damage to the fibre wrap under the surface enamel. • Check the cylinder for discolouration of enamel which can indicate heat damage. • Check the cylinder is free of water, oil and grease contamination, paying particular attention to the cylinder connection. • Check the main wheel valve is undamaged. ✓ Pass ✘ Fail Conduct a leak test:* • Connect the cylinder, demand valve and facemask. • Check that cylinder connection to the regulator is finger tight only. • Check the demand valve is parked. • Open the air cylinder half turn and allow the pressure in the system to stabilise. • Fully open the cylinder and turn back a half turn. • Listen for audible leaks or defects. • Close the cylinder wheel valve and observe the pressure gauge. • Check that the pressure does not drop any more than 10 bar in 60 seconds. For a pre-operational check, the contents of the cylinder should not empty in 30 seconds. ✓ Pass ✘ Fail Test the low air warning whistle:* • Open the air cylinder half turn and allow the pressure in the system to stabilise. • Fully open the cylinder and turn back a half turn. • Close the cylinder wheel valve and observe the pressure gauge. • Purge the air slowly at the demand valve and confirm that the low air warning whistle on the pressure gauge operates between 50 – 60 bar. ✓ Pass ✘ Fail If the leak test or low air warning whistle fails, check all the connections and repeat the test.
